Abstract

Air crash investigators use the Swiss cheese model to understand an accident’s cause. The model illustrates that although there are many layers of defense (slices of cheese) against an accident, there also are weaknesses (holes) in each layer. If the weaknesses align, a hazard can pass through and cause an accident. This model has been updated for organizational accidents.
