Abstract
Error-proofing systems help prevent defects from reaching customers, but automation changes can quietly weaken detection. This column describes a real-world incident in which a vision system logic update disabled a thread-presence check and allowed nonconforming parts to escape. The column summarizes the root causes and offers a lightweight, repeatable approach built on risk-based thinking and core tools to keep error-proofing effective through change.
