Abstract

A new root cause analysis (RCA) framework provides a simple method for finding a failure’s root cause. The framework includes a list of quality tools to use, a method for prioritizing hypotheses to investigate, a document to track investigation actions and a way to help ensure the underlying cause is identified. The framework is easy to implement and can be modified to fit an organization’s specific needs. Organizations that already have a problem-solving process can integrate the RCA framework into their existing process.
